NATIVE HEALTH is an Arizona State Department of Health Services Licensed Medical Outpatient Treatment Center.
NATIVE HEALTH provides primary medical care and is staffed by an M.D., RN’s, LPN’s and other professionals.
Services include:
• Well child, women and men check ups
• Immunizations and flu shots
• Family planning, pregnancy testing, pap smears
• Referrals to outside sources
• Acute and chronic disease management, acute illness, minor surgery (toenail removal, circumcision)
• Prenatal care
• Laboratory testing
• School physicals
NATIVE HEALTH is a Primary Care Program provider. The Primary Care Program is a program to increase access to community based primary (health) care services statewide to low income residents of Arizona with no health insurance. The Primary Care Program is funded by the Arizona Department of Health Services. It is based on a sliding fee scale. If you have been denied AHCCCS you may qualify for the Primary Care Program for an enrollment period of one year. The Primary Care Program is not insurance and does not cover specialty care, emergency services or hospital services. Clinic hours are Monday through Friday, 8:00 a.m.–5:00 p.m. (closed noon-1:00 p.m. daily). Limited walk-in medical appointments are available to registered patients Monday through Friday, 8:00-10:00 a.m. and 1:00-3:00 p.m. (except Friday afternoons). Free transportation is available in the metropolitan area (Higley to Surprise) on a pre-arranged basis.
NATIVE HEALTH has an Alternate Resource Representative. She is able to provide assistance with health care insurance, AHCCCS applications as well as food stamps (SNAP) and TANF. NATIVE HEALTH utilizes the Health-E-Arizona website to determine eligibility for medical, food/nutrition, and cash assistance programs. Please bring the following documents for your appointment: photo ID, social security cards, birth certificates, Tribal enrollment verification, proof of income and proof of residency. Appointments are available Monday-Friday, 8:00 a.m.-5:00 p.m.
Other programs include: Diabetes management (blood sugar control, glucose machine teaching, insulin management and diabetic nutrition) sexually transmitted disease prevention, testing and treatment, contraceptive care, HIV testing, school and sports physicals, and Health Start, a program to assist young mothers.
NATIVE HEALTH Primary Care Prenatal Services: NATIVE HEALTH is accepting Primary Care patients for prenatal and delivery care. The cost is $300.00 for all prenatal visits, one ultrasound and lab work. Delivery will be through emergency AHCCCS. Half of the fees are paid at the first visit and the remaining $150 the second month. Delivery will be at Phoenix Baptist Hospital. Car seat checks are also available at no charge.
