To make an appointment with the NATIVE HEALTH please call (602) 279-5262.
You may register at any time to become a medical patient of NATIVE HEALTH. If you would like to register for another program please call for more information on required documents. The registration requirements for the medical program are as follows:
1. New patients without Private Insurance or AHCCCS
a. Photo ID
b. Social Security Cards for household
c. Birth Certificates for household
d. Certificate of Indian Blood and tribal ID (if applicable)
e. Proof of income for past 30 days
f. Proof of residency
2. Every person eligible for IHS Title V medical benefits who wishes to receive services at NATIVE HEALTH Central is required to provide the following information when registering for clinic services:
a. Photo ID
b. Social Security Cards for household
c. Birth Certificates for household
d. Certificate of Indian Blood and Tribal ID
3. Eligible Verified Private Insurance/AHCCCS members served at NATIVE HEALTH Central or CHC West:
a. Photo ID
b. Proof of Insurance Coverage/AHCCCS Card
4. Potential Primary Care Program served at NATIVE HEALTH Central:
a. You must apply for AHCCCS
b. Proof of income (i.e., check stubs)
c. Birth certificates
d. Proof of Arizona residency (i.e., utility bills)
e. Arizona picture ID
5. Refugee Medical Assistance Program (RMAP) FFS to AHCCCS
a. Photo ID
b. RMAP Card
