Health Coverage and Family Assistance

NATIVE HEALTH’s Health Coverage Specialists are able to provide application assistance with AHCCCS (Medicaid), KidsCare, Supplemental Nutrition Assistance Program (SNAP), Temporary Assistance for Needy Families (TANF), or enrolling Health Insurance Marketplace. We use the Health-E-Arizona website to determine eligibility for medical, food/nutrition and cash assistance programs.

Please bring the following documents to your appointment:

  • Photo ID
  • Social Security cards (for individuals applying)
  • Birth Certificates
  • Proof of all income
  • Proof of housing and utilities – a utility bill
  • Bank statements or car titles (if applicable)
  • Tribal Enrollment Verification, if applicable

No appointment necessary. Applicants will be seen on first-come, first-serve basis. Appointments are available Monday through Friday, 8:00 a.m.–5:00 p.m.

Outreach Enrollment Specialists are available at NATIVE HEALTH Central, NHW Community Health Center, NATIVE HEALTH East, and the Information and Referral Center in the Metrocenter Mall.  Health Coverage Specialists are also for available community events.
