Health Coverage and Family Assistance

NATIVE HEALTH’s Family Health Advocates are able to provide application assistance with AHCCCS (Medicaid), KidsCare, Supplemental Nutrition Assistance Program (SNAP), Temporary Assistance for Needy Families (TANF), or enrolling Health Insurance Marketplace for new or established NATIVE HEALTH patients. We use the Health-E-Arizona website to determine eligibility for medical, food/nutrition and cash assistance programs.

Please bring the following documents to your appointment:

  • Photo ID
  • Social Security cards (for individuals applying)
  • Birth Certificates
  • Proof of all income
  • Proof of housing and utilities – a utility bill
  • Bank statements or car titles (if applicable)
  • Tribal Enrollment Verification, if applicable

No appointment necessary. Applicants will be seen on first-come, first-serve basis.
