How to work with a Family Health Advocate?

NATIVE HEALTH’s Family Health Advocates work alongside you to submit an application or renewal online. Appointments can be done in person, on the phone, or via Zoom. 

You can also begin to work or renew your AHCCCS application on your own and then reach out if you need extra help.  

How the process will work

There are four ways you can apply for healthcare coverage: 

  1. Apply through the online portal here
  2. Apply by phone at 1-855-432-7887 with HEA+
  3. Download & print application in English and Spanish
  4. Apply with our Family Health Advocates

Tips for after you apply:

  • You’ll get a letter in the mail if anything additional is needed. So, after you apply, check your mailbox often. The state will only correspond with you by mail.  
  • You will also get a reminder if it is time to renew your AHCCCS, SNAP, or TANF documents. 
  • Once you are approved, you can use your insurance as soon as your next appointment.
  • Once you get your results, be sure to provide your new insurance documents to the front desk at NATIVE HEALTH.
