New Patient Enrollment
You may register any time to become a medical patient of NATIVE HEALTH or NHW Community Health Center. We accept the following AHCCCS plans:

  • American Indian Health Plan
  • Care 1st of Arizona
  • Health Choice Arizona
  • Mercy Care
  • Mercy Maricopa Integrated Care
  • UnitedHealthCare
  • Comprehensive Medical and Dental Program (CMDP)

In addition, many private insurance plans are accepted. If you would like to register for another program, please call for more information on required documents. The registration requirements for the medical program are as follows:

New patients without private insurance or AHCCCS:

  • Photo ID
  • Social Security Cards for household
  • Birth Certificates for household
  • Certificate of Indian Blood and tribal ID, if applicable
  • Proof of income for past 30 days – a pay stub
  • Proof of residency – a utility bill

NATIVE HEALTH serves all patients regardless of inability to pay. NATIVE HEALTH offers discounts on the cost of services through the “Sliding Fee Scale (SFS)” program. Discounts for essential services are offered based on family size and income.

Every person eligible for IHS Title V medical benefits who wishes to receive services at NATIVE HEALTH is required to provide the following information when registering for clinic services:

  • Photo ID
  • Social Security Cards for household
  • Birth Certificates for household
  • Certificate of Indian blood or Tribal ID, if applicable

Eligible verified private insurance/AHCCCS members served at NATIVE HEALTH:

  • Photo ID
  • Proof of Insurance Coverage/AHCCCS Card

Refugee Medical Assistance (RMA) Program

  • Photo ID
  • RMA Card

New patients can save time during their first appointment by completing the New Patient Registration Packet prior to their visit.
