You may register any time to become a medical patient of NATIVE HEALTH or NHW Community Health Center. We accept the following AHCCCS plans:

NATIVE HEALTH serves all patients regardless of inability to pay. NATIVE HEALTH offers discounts on the cost of services through the “Sliding Fee Scale (SFS)” program. Discounts for essential services are offered based on family size and income.
Every person eligible for IHS Title V medical benefits who wishes to receive services at NATIVE HEALTH is required to provide the following information when registering for clinic services:
