NATIVE HEALTH accepts all Medicaid (also known as AHCCCS plans including American Indian Health Plan (AIHP)) in Arizona. NATIVE HEALTH accepts many private insurance plans including Aetna, Blue Cross Blue Shield, Delta Dental, Cigna and many others. We also offer a Discount Program for individuals who are uninsured or underinsured. NATIVE HEALTH also accepts Medicare at all locations. Evening and telehealth appointments available. NATIVE HEALTH can also help you apply for KidsCare. KidsCare is a free or low cost health insurance for families that meet income requirements. This covers medical, dental, and behavioral health services. Families can qualify for KidsCare by providing a photo ID, proof of income, and Certificate of Indian Blood or Tribal ID (if applicable), and Arizona residency. Notice of Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your health care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
  • If you schedule a health care item or service at least 3 business days in advance, make sure your health care provider or facility gives you a Good Faith Estimate in writing within 1 business day after scheduling. If you schedule a health care item or service at least 10 business days in advance, make sure your health care provider or facility gives you a Good Faith Estimate in writing within 3 business days after scheduling. You can also ask any health care provider or facility for a Good Faith Estimate before you schedule an item or service. If you do, make sure the healthcare provider or facility gives you a Good Faith Estimate in writing within 3 business days after you ask.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Please call (602) 279-5262, ext. 11035.
  • Make sure to save a copy or picture of your Good Faith Estimate and your bill.

New Patient Registration

To become a new patient of NATIVE HEALTH you must register and complete all necessary paperwork at the time of your first appointment. We accept AHCCCS (Arizona Medicaid), private insurance and we offer a Discount Program

For new patients with private insurance or AHCCCS, you will need to provide the following:

  • Photo ID
  • Proof of insurance (i.e. active private insurance card, AHCCCS card)
  • Certificate of Indian Blood or Tribal ID, if applicable

For new patients WITHOUT insurance or is underinsured you may be eligible for our Discount Program. To qualify for our Discount Program, you will need to provide the following:

  • Photo ID
  • Birth Certificates for children
  • Certificate of Indian Blood or Tribal ID, if applicable
  • Proof of income for past 30 days, for each household member (i.e. pay stub, other documents received as income)
  • Proof of residency (i.e. utility bill)

NATIVE HEALTH serves all patients regardless of ability to pay at NATIVE HEALTH Central, NHW Community Health Center, and the mobile medical unit NATIVE HEALTH offers discounts on the cost of services through the Discount Program. Discounts only apply for essential services.

Federally Recognized American Indian/Alaska Natives may have waived services for Medical and Behavioral Health with an active application for the Discount Program.  They  must provide the following information at time of registration:

  • Photo ID
  • Birth Certificates for children
  • Certificate of Indian blood or Tribal ID, Tribal Enrollment Card, or Tribal Resolution or Enrollment Letter

If you are part of the Refugee Medical Assistance (RMA) Program, you will need to provide the following:

  • Photo ID
  • RMA Card

NATIVE HEALTH is introducing online check-in for your appointments!  It’s easy, will save you time, and make your visit more streamlined. Phreesia is an online platform that updates your check-in information before a medical, dental, or behavioral health appointment. You will receive an email and/or text before your scheduled appointment. It’s safe and secure! 

If you encounter any problems with this system, need extra assistance, or can’t access the email or text, our staff will happily help by phone or in-person at our offices. 

New Patient Registration
New patients can save time during their first appointment. When scheduling your first appointment, you will receive a secure link to Phreesia to complete your New Patient Packet. This can be done on your phone, tablet, or laptop. If you are unable to download the link or have difficulty, we can assist you in the clinic when you arrive. 

Please plan on arriving 45 minutes early for your first scheduled appointment.  If you have any previous medical records, immunization records, or are taking any medications please bring them to your appointment.
