There are four ways you can apply for healthcare coverage: 

  • Apply through the online portal here
  • Apply by phone at 1-855-432-7887 with HEA+
  • Download & print application in English and Spanish
  • Apply with our Family Health Advocates

Tips for after you apply:

  • You’ll get a letter in the mail if anything additional is needed. So, after you apply, check your mailbox often. The state will only correspond with you by mail.  
  • You will also get a reminder if it is time to renew your AHCCCS, SNAP, or TANF documents. 
  • Once you are approved, you can use your insurance as soon as your next appointment.
  • Once you get your results, be sure to provide your new insurance documents to the front desk at NATIVE HEALTH.
