NATIVE HEALTH’s Emergency Assistance Program

Do you, or anyone you know, have a family medical emergency in the Phoenix metro area? If so, NATIVE HEALTH’s Emergency Assistance Program is here to help you. This program is for Native Americans who reside on a reservation and have a family member that needs emergency medical services in Phoenix. NATIVE HEALTH’s Emergency Assistance Program can provide some financial assistance which may help with food, lodging, necessities and gasoline.

To access the program, you will need a statement from the hospital social worker or any physician. The letter must contain the patient’s name, diagnosis and that the family member needs to be in attendance. Upon discharge, additional financial support is available for their travel home. Additional documents are required.
