Public Health Nurse Case Management Services

Public Health Nurse Case Management Services (PHN) is now accepting clients at this time for PHN-Case management services. The program is geared towards supporting the Native American Community by connecting patients to specific resources that will help address the social determinants of health to prevent barriers to care and gaps in services.

Eligibility requirements:

Native American/Alaskan Native
Ages 10-35 years old
Must be actively enrolled or in the process of seeking treatment within BHD
Have co-occurring disorders

The program helps patients meet the following needs:

Housing Resources
Transportation
Food security
Employment Resources
Care Coordination
Client Home Visits
Health Education
Medication Reconciliation
