The Arizona Long Term Care System (ALTCS) Tribal Case Management Program

The Arizona Long Term Care System (ALTCS) Tribal Case Management Program, a division of the Arizona Health Care Cost Containment System (AHCCCS), provides acute care (general medical care services), behavioral health, long term care, and case management services.

These services are offered to American Indian ALTCS members who are at risk of being institutionalized (the elderly and/or those who have physical disabilities) and who reside on a tribal reservation.

American Indian ALTCS members residing on the tribal reservations listed below receive case management services through NATIVE HEALTH (Native American Community Health Center, Inc.). Tribes served by NATIVE HEALTH include the following:

  • Ak-Chin Indian Community
  • Cocopah Tribe
  • Colorado River Indian Tribes
  • Fort McDowell Yavapai Nation
  • Fort Mohave Indian Tribe
  • Havasupai Tribe
  • Hualapai Tribe
  • Kaibab-Paiute Tribe
  • Quechan Tribe
  • Salt River Pima-Maricopa Indian Community
  • Tonto Apache Tribe
  • Yavapai Apache Nation
  • Yavapai Prescott Tribe
